The PDE-3048 is now open and due at 4:00 PM on February 5, 2025.

​​Changes for 2024 PDE-3048 - 

Do not include students who have been placed by a medical provider, parent, guardian, or other family member.  

You must enter how the student was placed in the Note field.   

  1. Court adjudicated, or placed as the result of

  2. Abandonment,

  3. Neglect, or

  4. Death of their parents or guardian.

Due to the delay of the PDE-3048, the reporting system will be closed for reporting of student served for the 2024-2025 school year.  All users will be notified via email (from nanddcommittee@gmail.com) when the system is open.
